▓ What supporting materials shall I submit when applying for reimbursements ?

1. All the "receipts" for medical treatment. (Not the statement of details. If you submit a copy, you should have the hospital stamp it.)

2. A "medical certificate of diagnosis" issued by the doctor. If you submit a copy, you should have the hospital stamp it.

3. Fill in the "Insurance Proceeds Application Form" and a consent letter for use of personal information (downloaded from the Center's website)

4. A photocopy of the cover of your bankbook.

5.A photocopy of your student ID card (front and back)

Students shall bring the above materials to the Health Promotion Center to apply for reimbursements. Applications should be filed within two years of the event lest the students’ rights be affected.
